The need for bone grafting beneath the elevated sinus membrane remains widely debated. The purpose of this study was to compare the evolution of implant stability over 5 years in grafted versus non-grafted sites.\n\nMaterials and Methods:\nA total of 12 patients (9 women and 3 men) with an initial bone height (IBH) \u2264 4 mm in posterior maxillary sites were included. Implants (n = 37) were placed using the OSFE technique. According to randomization, the sinuses received either a bone graft (n = 20, control group) or no graft (n = 17, test group). Some patients received both treatments when both of their sinuses met the inclusion criteria. Follow-up evaluations were performed at 1 week, 10 weeks, 12 weeks, then at 1, 3, and 5 years after implant placement. Periapical radiographs were taken, and the Implant Stability Quotient (ISQ) was measured at different time points.\n\nResults:\nAt placement, the mean ISQ was 58.9 \u00b1 11.2 for the test group and 53.8 \u00b1 10.2 for the control group; it dropped at 10 weeks post-placement before increasing afterward. Five years after placement, the mean ISQ reached 80.8 \u00b1 4.2 in the control group and 79.7 \u00b1 4.3 in the test group. The difference between groups was not significant.
